Adult Preventive Plan Per Year ($400.00)
- 2 teeth cleanings
- 1 Exam
- 1 X-ray if needed

- 10% discount on other select dental treatments
- Fluoride treatment
This plan means a $200.00 minimum savings per year!
Periodontal Savings Plan Per Year ($625.00)
- 1 Exam
- 1 X-ray if needed
- 3 Periodontal cleanings
- Emergency exams
- CBCT (patient-specific)
- Photos and consultations
- Radiographs
- 10% discount on other select dental treatments
- 10% discount on additional periodontal treatments
- Fluoride treatment
This plan means a $268.00 minimum savings per year!
Child Preventive Plan Per Year ($350.00)
- 2 teeth cleanings
- 1 Exam
- 1 X-ray if needed
- Emergency exams
- CBCT (patient-specific)
- Photos and consultations
- Radiographs
- 10% discount on other select dental treatments
- Fluoride treatment
This plan means a minimum savings of $200.00 per year!
Limitations and Exclusions
- The annual enrollment fee is due at the time of enrollment.
- Annual enrollment fee premiums must be paid with cash or check.
- CareCredit may not be used to pay a premium or patient portion of the treatment.
- The plan is effective immediately following paid enrollment.
- No refunds will be issued.
- The plan will only remain active if all additional payments are made at the time of service.
- Cannot be combined with other offers or plans.
- All fees shall be based on our practice’s UCR fees.
- For emergency visits, only exams and radiographs are covered—the patient is responsible for paying for any additional work.
- If a patient is diagnosed with periodontal disease, they are not eligible for the preventive plans. If during the plan year, the patient is diagnosed with periodontal disease, the plan will be prorated toward the Periodontal Health Plan.
- Smile Maker Savings Plan is only accepted at Fox View Dental—therefore if the patient is referred elsewhere benefits will not carry over.
- Smile Maker Savings Plan discounts are not applied toward products or facial aesthetics—teeth whitening, electronic toothbrushes, etc.
- Child Preventive Plan is for children aged 3 to 13.
